Accepted Insurances
Mt. State ENT accepts most major insurance plans, including, but not limited to: Aetna, BlueCross BlueShield, Carelink/Coventry, Cigna, Medicare, Medicaid, WellsFargo.  If you have a question regarding the participation of our office with your insurance, please contact your insurance company.

Referrals
Certain insurance companies may require that a written or an electronic referral be on file prior to your visit with our office. It is the responsibility of the patient to know if a referral is required and to initiate and obtain any required referral from the primary care physician.

Payments Required at the Time of Service
Copayments, coinsurance, deductible and payment for non-covered services are due at the time of service.  Mt. State ENT accepts VISA, MasterCard, Discover, Care Credit, cash and check.

No Insurance
If you do not have medical insurance coverage, payment is due at the time of service.  Our office does offer a discounted fee schedule for patients with no medical insurance coverage.

Monthly Statements
Patient statements are issued after the insurance company (if applicable) has processed the claim for services that have been rendered.  The statement will show any open and current balance due.  There may also be a reason for denial from your insurance company.  Statements are printed and mailed in 30-day cycles and payment is due upon receipt of the statement. 

Mt. State ENT does offer payment plans on accounts with an active balance.  The minimum payment accepted on most accounts is $25.00. 

Our office offers a discount for payment made in full on any account greater than $50.00.  For more details on this discount or to take advantage of this discount, please contact the billing office at (304) 255-2341 x 315.
